Physician offices and hospital outpatient clinics: please use HCPCS Code J9041 (injection, bortezomib, 0.1 mg)
Note:
The billing unit for code J9041 is 0.1 mg. For a 3.5-mg single-dose vial of VELCADE, the total number of billing units would be 35.
